Sanosuke Sagara is a former member of the Sekihō Army. When the group was destroyed by the Meiji government, he became a fighter-for-hire to calm his anger by fighting. During his introduction in the series, he encounters the wanderer Himura Kenshin, who easily defeats him and can convince him to stop his mercenary work and instead start protecting people. Sanosuke becomes Kenshin's best friend and his partner in most of their fights.

He watched Kenshin meditate in the distance, thinking about how far he had come since those bloody days in the mountains. The scars on his arms throbbed in the cold night air, reminders of a life left behind. Yet, for the first time, silence felt like peace rather than loneliness.

Sanosuke's journey begins when he clashes with Kenshin at Kaoru's dojo, initially seeking a fight but finding purpose instead. His past as a mountain bandit leader is revealed through flashbacks during the Kyoto Arc, showing his isolation after losing his comrades to government forces. During the Kyoto conflict, Sanosuke fights alongside Kenshin against the Juppongatana and Enishi's assassins. His combat skills with the naginata prove vital in protecting friends from overwhelming odds. In the final Hokkaido Arc, Sanosuke confronts the remnants of the Shishio group and Enishi's forces.
